Anti-Coagulants in Atrial Fibrillation

GP Federations aim to provide better care, delivered in a more responsive way and closer to home, for patients registered on the lists of practices within the Federation. The focus is on working across the local health and social care community, in collaboration with a wide number of agencies, to design and implement innovative healthcare strategies and ways of delivering high quality care. The  GP federation is developing a Federation Enhanced Service to identify and treat patients who are diagnosed with AF but not appropriately anticoagulated.

Primarycare and Community Together (PACT) is an innovative new social enterprise

which delivers locally targeted patient centred health initiatives that address some of the most entrenched health issues facing society. PACT is committed to developing  interagency involvement to enable a maximum beneficial outcome. PACT will promote closer co-operation and collaboration on the development of new and innovative approaches to improve patient care and meet the challenges facing the delivery of health and social care. PACT will use the unique network of community pharmacy outlets to deliver programmes in partnership with local host communities.

  • This project will  identify people who have been diagnosed with AF and who are not appropriately antigoagulated with the purpose of: 

Initiating an appropriate anticoagulant 

OR

Recording the reason why this management is not suitable for them

  • This project will also provide training to pharmacists within the locality to better equip them to provide support to people affected by AF who are taking part in this project. This training will include increased knowledge of medication support and common adherence issues.

 

  • Patients would be supported by their community pharmacy to ensure their understanding, compliance, and provide further information regarding their medicine. This would be part of the patients long term management of AF.

PACT will identify all the community pharmacies within the locality of the GP practice, provide a training process (in partnership with ABPI) for the community pharmacists involved which will support the pharmacies to provide a Medicines optimisation brief intervention (MOBI) for patients who have been recruited onto the project. The training programme would include knowledge of, brief intervention skills, medication support in AF and common medicines compliance issues. Training materials and courses provided by NICPLD will also be explored. The community pharmacy involvement within the project will be facilitated by PACT (Primary Care and Community Together)
